Pattern Reference
Hepatocellular - R ≥ 5
ALT elevated >2× ULN with minimal ALP. Higher risk of ALF. Common: INH, acetaminophen, diclofenac.
Cholestatic - R ≤ 2
ALP elevated >2× ULN, jaundice, pruritus. Better prognosis. Common: amoxicillin-clavulanate.
Mixed - R 2–5
Both ALT and ALP elevated. Common: many antibiotics, phenytoin.
Steatohepatitis / Microvesicular
Diagnosed by histology, not R ratio. Mitochondrial toxicity. Valproate, tetracycline (IV).
